Alternatively, if you do not have access to a mobile device, are having trouble using the online registration form or would prefer not to register digitally, then please call in to reception (between 8:00am and 6:00pm Monday to Friday) and pick up one of our registration packs or you can download one below.
If you have any repeat medication, please provide specific details of this on the online registration form; or if you are not registering digitally please provide us with your re-order list (the white part of the prescription slip) from your previous surgery.
